Annual statement instructions

To satisfy the Oregon annual statement filing requirements you must:

  1. File your annual statement electronically with the National Association of Insurance Commissioners (NAIC) according to your domicile state’s instructions.
  2. Send a hard copy of the signed jurat page as proof of filing to:
